Patient Registration

To get a state issued ID card, you need to visit a doctor, Dr Grisales is licensed to prescribe medicinal marijuana. Dr Grisales will want to know your medical history and might need medical records from previous doctors that include information relevant to your qualifying condition. Make sure you are able to access your medical records before making an appointment.

Application requirements will ask for basic information, like your name, address, and phone number. It will also usually ask for a variety of information related to your condition and medical history.

Patient Appointment

In order to get this medication or get certified to receive this medication the patient first has to make an appointment where the physician will speak with him/her and do a history on present illness and basically determine if indeed the patient will benefit from the medication or the patient wont. If he feels that the patient won’t benefit from the medication the patient won’t be placed on the cannabis treatment. But if he feels that the patient will benefit from the medication based on the patients individual criteria then he will prescribe the cannabis treatment.

Patient Registration

Once the patient has been seen by the physician and he looks at the history on patients present illness he has to add the patient to a registry. Your doctor to fill out an application and submit it to the state for approval. Once the patient is added to the registry, what’s understood is that the patient is added to a university that’s undertaken a research and the patient is assigned a specific number. Once the physician gives the patient that number that has been assigned to him/her.

Prepare payment.

There are fees associated with Doctor visit. Typically, you must cash pay. Full fees in first visit to $250 and follow up $150. While medical marijuana is legal in certain states, it remains illegal on a federal level. Bank cannot accept check or credit card charges.

Patient Call to

The patient then has to call the registry and follow up with them. At this point the doctor has no control of the circumstances that the registry may give the patient. Once the patient gets the identification number and gives the registry that number and a valid Passport ID , the registry will then ask for the patient to send a 75$ payment for the certification. Once they receive all the patients information and it has been put on the system.

They will then contact the patient via email or via phone to let the patient know he/she is eligible to receive the medical cannabis. You may log in using your email address once it has been added by your physician. With the certification, the patient is now able to go to a dispensary. Once the patient shows their certification they will then pull up the patient information on their system and on their system is where the prescription will appear. So keep in mind this is not a physical prescription like on a piece of paper. This is all done systematically and will appear at the dispensary (where the medical cannabis is being hold).

Now, the medications do come in variety of forms. They come in tablets that can be swallowed, chewable, and CBD oil. The oils are in a vape form so it does require the patient to inhale. The unit that has the CBD oil, releases a specific amount based on the doze that was provided to the registry. Once the patient has done the process to be able to receive the medical cannabis, the patient still has to follow up with the physician up to every 3 or 4 months depending on what the regulations are at that moment in time.

Regulations do change because its relatively new. But it does require the patient to do follow ups to renew their certification and to adjust the amount of medication the patient is receiving, if it is just.

RENEW THE CARD To maintain an active Medical Marijuana Use Registry identification card, a patient and/or legal representative must annually submit a renewal application, along with the application fee and any required accompanying documents to the department forty-five (45) days prior to the card expiration date.

STOLEN OR REPLACE CARD Requests to replace a lost or stolen card will require the cardholder to submit a Change, Replacement or Surrender Request Form, along with a copy of his or her Florida driver’s license or identification card and a $15 replacement fee.
